Online Courses Group 3 - Critical Requirements

Before registering, there are 4 simple steps to be followed:

Completion Affidavit – MUST PRINT AND RETURN TO RECEIVE CE CREDIT When you complete your online courses, you will need to fill out a course completion affidavit and fax or mail back to LIBOR. You must complete and return the affidavit in order to receive CE credit. Fax to LIBOR at (631) 661-2103 or mail to LIBOR at LIBOR, 300 Sunrise Hwy, West Babylon, NY 11704 Attn:OnLine Education. System Advisement – QUICK SYSTEM CHECK BEFORE YOU BEGIN If you currently have any type of POP-UP Blocker Software installed in your PC, you will not be able to access these courses. If you would like to continue with LIBOR's Online Courses you will need to uninstall the program before beginning your class.
