Before registering, there are 4 simple steps to be followed:

Affidavit – MUST PRINT AND RETURN
TO RECEIVE CE CREDIT
When you complete your online courses, you will need to fill out a course completion
affidavit and fax or mail back to LIBOR. You must complete and return the affidavit
in order to receive CE credit. Fax to LIBOR at (631) 661-2103 or mail to LIBOR
at LIBOR, 300 Sunrise Hwy, West Babylon, NY 11704 Attn:OnLine Education. You
can print your affidavit here.
